Survey Field Types

Core Fields
single line text
The single line text field allows you to collect open-ended responses.
Matrix

The single line text (matrix) is very similar to the single line text field, except it allows you to collect multiple answers for one question. This is especially useful for related questions. For example, you could ask for their name, age, address, etc. with this one field.
multiple choice
With the multiple choice field, respondents can select multiple options to the question. For example, you can ask what type of foods they like and allow multiple answers.
Matrix

This field takes the standard multiple choice field and puts it into a grid, allowing you to ask multiple questions while also collecting multiple answers.
single choice
You can have your respondent select one option from a predefined set of choices with the single choice field. For example, you could ask their favorite color.

The single choice (matrix) is just like the multiple choice (matrix) field, except it allows just one select for each question.
Additional Fields
paragraph text
If you need to collect open-ended responses that are longer than just one line of text, the paragraph text field is perfect for you. Common uses are questions requiring explanation, or extended feedback.
new section
It's often important to separate content on a page, or sometimes an extended explanation is needed. The new section (also known as section break) is great, and it also allows HTML so you can style it as you wish. This field is also great for use as a "welcome message".
image
Spruce up your survey and add your logo with the image field. You could also display the image, and collect responses based on the picture.
file upload
By placing a file upload field on your survey, you can allow respondents to upload files while taking your survey. For example, they could upload their resume while filling out the job application. The maximum size of an individual file upload is 5 MB.
